Once you have made your initial appointment with our office, pre-register by printing and completing all of the General Consent Forms and please complete both forms under the Speech Language Therapy Tab OR the Occupational Therapy Tab respective to your scheduled therapy evaluation. Completing these forms prior to your visit will help speed up the new patient registration process.

Please be sure to bring all the forms to your child’s first visit; along with, a copy of your child’s health insurance card and any other pertinent information (e.g. prior speech and language evaluations or records). These forms can also be emailed in advance to info@pediatrictherapylinks.com.
